Respiratory Nurse Specialist Team (Adult)

WHO WE ARE

We are a team of registered nurses who have specialised in managing and treating a variety of chronic lung conditions.  We work alongside a multi-disciplinary team and the respiratory consultant Dr Hamdi Amar.  The team is based at the Jersey General Hospital, providing a service to patients throughout Jersey. Visiting patients in their home or in clinic. Supporting patients in a holistic manner to prevent hospital admissions where possible and supporting their discharge.

Services offered to our patients:

  • In Control Group at the Hospice – Helping patients to manage Breathlessness and panic attacks. Need GPs or Nurses referral
  • Free Breathlessness Support Group – Happens on the 3rd  Thursday of the Month between 1pm to 2pm. Bus number 6 from Town centre to Hospice
  • Breathlessness Rehabilitation Program at Springfield Stadium and St Bernadette in Quennevais Sports Centre. Referral via Hannah Roberts – Physiotherapy Department
  • Silver Line – Free Phone support for Elderly people – 0800 470 8090
  • I believe there is a “Man Shed Group” for Men to have support

WHAT THE SERVICES OFFERS:

An expert service to support adults who have difficulties with breathing, as a result of a wide range of chronic lung conditions.  A holistic review of the patient, including a review of their social and medical needs and referring to other services as appropriate. A pharmalogical review of their respiratory medications 

Optimising inhaled therapy and providing guidance, ongoing support or assisting with trials.  Including ongoing support for those on nebulised therapy and or nebulised antibiotics. 

We are able to provide education and self-management to support distressing symptoms such as acute breathlessness.

Provide nursing support for panic, anxiety, depression Advice on managing their condition and exacerbations Supporting end of life care planning and decision making Ensuring Asthma management plans are in place and understood Respiratory

WHO TO REFER

Difficult to manage COPD and Asthma patients

COPD moderate to Very Severe (m) MRC dyspnoea ≥ 2

Respiratory patients requiring Long Term Oxygen Therapy (LTOT)

COPD patients with repeated exacerbations, infective and non-infective

All adult asthma admissions or ED attendances Pulmonary Fibrosis (IPF)

Difficult to manage Bronchiectasis

WE DO NOT SEE UNDIAGNOSED LUNG CONDITIONS, PNEUMONIA OR LUNG CANCER PATIENTS SERVICE AIMS:

Review referral and aim to see patient within 1-2 weeks depending on need Follow-up of referred patients within 2 weeks of discharge from hospital if admitted with acute respiratory exacerbations
